The board issues the licence, and decides a good deal for itself
A state medical board issues the licence. It sets its own requirements on top of the national examinations: the postgraduate years, the background and disciplinary checks, the documents it wants verified, and how it treats an application from somebody trained elsewhere.

What it is for
People reason about medical licensing as though it were one system, because the examinations are one system. The licence is fifty systems, and the differences are not small print: they are the number of years, the documents, and sometimes whether an applicant in your category is eligible at all. The board is also the body that can act on a licence later, which is why its rules are worth knowing before you need them rather than after.
How to think about it
Read the board's own page for your category, and list what it wants verified and by whom, because most of those are other people's queues. Then ask which item on the list has the longest lead time and start that one first. Which of these depends on somebody other than me? Boards set these rules themselves and change them, so any figure you plan around has to come from your own board's page rather than from here, from a colleague, or from a summary written in another year.
